CWM-TRK Forms
ATTENTION: As of January 1, 2008, anyone submitting any of the CWM-TRK forms listed below must have completed the UIUC Chemical Waste Requirements Training prior to form submission. To complete this training requirement, please visit http://www.drs.uiuc.edu/css/training. The purpose of this requirement is to ensure all personnel who manage chemical waste are aware of and understand the requirements set forth by the Environmental Protection Agency (EPA) which pertain to chemical waste collection, storage and disposal in labs and other generation areas, as well as procedures for the UIUC Chemical Waste Program.
The forms below will need to be printed, signed, and then sent through campus mail to DRS.
CWM-TRK-01 - For chemicals and mixtures that already have a UI#:

CWM-TRK-02 - For chemicals that do not have a UI#:

CWM-TRK-03 - For mixtures that do not have a UI#:
